Children's Hospital of Philadelphia is seeking a Nurse Navigator for their Refugee Health Program.
The Nurse Navigator for CHOP's Refugee Health Program supports research, quality improvement, clinical, and other programmatic activities related to CHOP's role as the medical home for medically complex refugee and humanitarian entrant patients who predominantly speak languages other than English. This includes families who speak 30+ languages other than English, families who are pre-literate, and families without prior familiarity with the US health system.
With supervision from the Refugee Health Program staff, they are responsible for IRB submissions, medical record review, data reporting/monitoring, and care management. They play a core role in scholarly, quality improvement initiatives: Specifically, they assist patients in navigating the health system, attend navigation-related meetings, and document information (generally in REDCap) for navigation program improvement. For a subset of Refugee Health Program patients, they will serve as a liaison between primary care providers (PCPs), outpatient specialty care providers and inpatient care teams, as well as payers, medical equipment suppliers, home care providers, subacute and chronic care facilities, community resources (e.g., schools, Early Intervention, refugee agencies), and relevant government agencies (e.g., PA Dept of Health).
Additionally, they will collaborate with Refugee Health Program leadership to draft grants and proposals relevant to the above services. They will work effectively with in person, video, and phone interpreters. Proficiency in more than 1 language is preferred but not required, as is experience working in multicultural settings.
This role serves as the primary clinical contact for patients and families seeking certain specialized services at The Childrens Hospital of Philadelphia that require an additional focus on care coordination. This role is charged with assisting new patients in understanding their care plan, coordinating communication between multiple care team members across the care continuum; participating in family meetings; providing patient/family education, and providing care coordination including scheduling appointments and telephone triage. The role may also assist with development of outreach and research activities for their department. Triages/ manages incoming new and established patient calls/email requests. Assists with the collection and review process for all potential patients. Works in collaboration with the clinical staff (Medical team, Physical therapy, Social Work, etc.), Out of Area Specialist, Business Offices, to ensure all aspects of the patient and family care coordination results in an ideal patient experience.
