Description: The CareSight Nurse Practitioner and Physician Assistants lead a care team that consists of Registered Nurses (RN) and Community Health Workers (CHW). They work closely with the patients' primary care physician. Position Essential Functions:
- Provides in-home medical care to identify and treat health issues in collaboration with the patients' primary care physician
- Leads team that includes RNs and CHWs
- Collaborates with RNs and CHWs in the development of an integrated patient centered care plan
- Supports patient and caregiver goals
- Communicates effectively with entire care team including the patients' community primary care provider
Qualifications
- Travel required
- Required:
- Must be fully COVID vaccinated
- Holds active, unencumbered New York State license
- Active DEA license
- At least two years of clinical experience delivering care to complex patients preferred
- Active, unrestricted New York State driver's license
