Bargaining Unit: UUP Job Summary: The Department of Transplant Surgery at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner. The successful candidate will:
- Attend post-transplant clinic and perform independent clinical assessments of transplant recipients.
- Review laboratory, radiographic and diagnostic test results; monitor for adverse effects and changes which could affect the patient.
- Collaborate with attending surgeons and neurologist related to results and plan of care.
- Maintain continuity of care between inpatient and outpatient services.
- Provide patient education related to post-transplant care.
- Develop and implement protocols for health maintenance, screening and quality of life management after organ transplantation.
- Create referrals to subspecialty care as needed.
- Participate in multidisciplinary care coordination.
- Participate and assist with quality improvement initiative and programs.
- Perform additional duties/responsibilities as required and/or needed by the department.
Required Qualifications:
- New York Registered Nurse and Adult/Family Nurse Practitioner Licensures.
- Master's Degree in Nursing.
- 10+ years of nursing experience.
- Current national certification as an Adult/Family Nurse Practitioner.
Preferred Qualifications:
- 3+ years of experience as a Nurse Practitioner in an outpatient setting.
- Prior experience in the care of chronically ill patients.
