Position Summary

The Nurse Practitioner will be required to perform consults and follow-up visits with palliative care community patients primarily in the home, the virtual environment and in the office setting. The position will require home visits/travel to the following areas: East Hampton, Southampton, West Hampton, Riverhead and South Hold townships. The position includes flexible full-time hours and the ability to work from home one day per week for administrative work and/or virtual visits. The Nurse Practitioner promotes cooperation, a positive attitude and creates a safe environment for both patients and personnel. He/she is responsible for knowing and adhering to established hospital policies and procedures, including all aspects of safety.

Duties of a Nurse Practitioner may include the following but are not limited to:

  • Collaborates with primary care physicians and specialists, licensed home care agencies and the palliative care social worker to coordinate palliative patient management.

  • Performs home and office procedures as warranted including home IV hydration and vaccination.

  • Aids the palliative care physician in following results and working in a collaborative nature with the interdisciplinary team.

  • Provides comprehensive symptom management and advance care planning with the patient and conduct family meetings with the social worker.

  • Performs administrative and clinical tasks to support the work of palliative care in the community.

  • Participates/collaborates in symposium presentations, staff and wellness community events at the hospital and throughout the community.

Required Qualifications: 

  • Current licensure to practice in New York State

  • Nurse Practitioner Master's in Nursing degree

  • Minimum 5 years experience as general Nurse Practitioner or three years dedicated experience in the field of palliative care and/or hospice care

  • Strong desire to work in a community palliative role

  • BLS and ACLS certifications through the American Heart Association

  • ANCC certification (or equivalent)

  • Driver's license and ability to travel independently

  • Standing/walking 75% of the time

  • Ability to occasionally lift heavy objects

Prior to start date, the selected candidate must meet the following requirements:

  • Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Southampton's Employee Health Services*
  • Successfully complete a 4-panel drug screen*
  • Meet Regulatory Requirements for pre-employment screenings.
  • Provide a copy of any required New York State license(s)/certificate(s).

Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.

*The Human Resources department will be responsible for any fee incurred for examination.
