Job Summary :
We are seeking an experienced Nurse Practitioner with Endocrinology expertise to join our multispecialty group in pursuit of high-quality patient care to provide full scope Endocrinology services in collaboration with our physicians.
Responsibilities :
* Review patient's history, reports and examination results. Perform physical examination. * Order and interpret appropriate diagnostic tests to determine diagnosis and design individualized treatment plan. * Evaluate and diagnose patients with a variety of hormonal diseases and conditions related to the endocrine systems. Provide appropriate treatment and patient care. * Provide comprehensive cost-effective diabetes care with available support of CDE(Certified Diabetes Educator)/RD (Registered Dietitian) services. * Recommend suitable management for the patient, including but not limited to, preventive health recommendations and habits. * Prescribe medications in accordance with New York State and federal law and formulate ongoing treatment and disease management plans. * Maintain complete and accurate medical records. * Provide relationship-centered care, taking into account the patient's psychosocial and physical needs. * Communicate effectively with the clinical team to ensure total delivery of quality care. * Perform other job-related duties as required. * Participate in medical staff educational programs and meetings.
The Ideal Candidate :
* One to three years of endocrinology clinical experience in an outpatient setting. * Ability to diagnose and treat a wide variety of endocrinology conditions with a focus on improving patient outcomes. * Demonstrate competency in diagnosis and treatment to assure high standards of patient care and safety are maintained. * Patient-centered approach toward a positive patient experience. * Excellent communication skills. * Interest in Population Health Management.
Qualifications :
* Master of Science degree in Nursing (MSN) * Graduate of a Nurse Practitioner education program registered by the New York State Education Department. * Certification as a Nurse Practitioner from a New York State approved accrediting body - American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP). * Must possess current, unrestricted New York State Registered Nurse license. * Seeking experienced clinicians with Endocrinology or Diabetes treatment outpatient experience- Two to three years strongly preferred. * Must possess active Drug Enforcement Agency (DEA) registration or be eligible for registration. * Must possess active Medicare/Medicaid Enrollment or be eligible to enroll. * BLS or ACLS certification required. * Must possess strong communication skills both oral and written. * Comfortable with the use of Electronic Medical Record (EMR) system and Microsoft Office.
