As an Family Medicine Nurse Practitioner in our outpatient department, you will play a pivotal role in delivering comprehensive healthcare services to our patients. You will diagnose and treat a wide range of medical conditions, provide preventive care, and promote overall health and well-being. Key Responsibilities:
Patient Care:
- Conduct thorough medical evaluations, examinations, and assessments for outpatient clients.
- Diagnose and treat a variety of acute and chronic illnesses.
- Develop and implement individualized treatment plans.
- Provide preventive care, health screenings, and vaccinations.
- Manage and monitor chronic conditions, including diabetes, hypertension, and more.
- Offer counseling on lifestyle modifications, nutrition, and exercise.
Medical Records and Documentation:
- Maintain accurate and up-to-date electronic medical records.
- Document patient encounters, diagnoses, treatments, and progress notes in accordance with medical standards and regulations.
Collaboration and Consultation:
- Collaborate with other healthcare professionals, including physicians, nurses, specialists, and support staff, to ensure coordinated patient care.
- Consult with specialists as needed and facilitate referrals for further evaluation and treatment.
Patient Education:
- Educate patients and their families about their medical conditions, treatment options, and preventive measures.
- Encourage and empower patients to actively participate in their healthcare decisions.
Quality Assurance:
- Adhere to evidence-based guidelines and best practices to deliver high-quality care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Continuing Medical Education:

Qualifications:
- Master's degree in Nursing (NP)
- State licensure as an NP Family Medicine.
- Strong clinical skills with a commitment to evidence-based practice.
- Excellent communication and interpersonal skills.

- Ability to work effectively and collaboratively in an outpatient healthcare team.
