Identifies anddiagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.Consistent with the established nursing practice, differentiates between normal and abnormal findings inthe screening process to detect changes in health status that require physician's intervention.Develops and maintains partnerships with community-based organizations and makes referrals toconsulting physicians and/or other health care professionals and community agencies, as necessary.Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.Consults with physicians and other health care practitioners in the evaluation of health screening findings tomake prospective decisions about treatment that will best support improved outcomes and patient satisfaction.Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursingassessment and other pertinent information. Identifies health and medical risks and needs.Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as perclinical standards and practice protocols, where needed, to achieve medical and health goals.Orders, conducts, and interprets diagnostic and laboratory tests.
