Position Summary
The Advanced Heart Failure and Transplant team is growing, and we are seeking an experienced Nurse Practitioner (NP) with Ventricular Assist Devices (VAD) experience to lead our VAD program. The Nurse Practitioner will work collaboratively with a multidisciplinary team including but not limited to others on the Advanced Heart Failure Nurse Practitioner team, Cardiac surgeon, Heart Failure/MCS/Transplant Cardiologists, Program admin team, and Social Work team to provide optimal patient care.
Responsible for direct patient care and coordination within the Advanced Heart Failure/VAD program. In addition to below, responsibilities include:
