We're seeking a dedicated Nurse Practitioner to join our care team in Buffalo, NY. As a Nurse Practitioner with Emcara Health, you'll play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey.
What We Offer:
- Competitive Pay: $108,000 - $140,000 per year
- Sign-On Bonus: $30,000
- Flexible Scheduling: Work on your own schedule, with a minimum of 25 visits per week

Key Responsibilities:
- Conduct comprehensive in-home assessments and develop individualized treatment plans
- Manage and monitor chronic health conditions
- Prescribe medications and educate patients on proper administration
- Promote preventive healthcare measures
- Collaborate with physicians, nurses, and other healthcare professionals
- Maintain accurate and up-to-date electronic health records
Qualifications:
- Active, unrestricted current licensure as a Nurse Practitioner in the state of New York
- Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
- Obtain autonomous provider status within six months of meeting eligibility requirements
- Preferred two years of experience in a primary care or longitudinal care setting
- Active driver's license and current Basic Life Support Certification
Preferred Qualifications:
- HEDIS/STAR measures experience
- Home-based visit experience
- HCC documentation experience
Physical Requirements:
- Ability to navigate stairs and a variety of dwelling conditions
- Ability to sit, stand, and kneel to perform physical assessment

Schedule:
- Standard business hours: Monday to Friday, 8am - 5pm, with a one-hour lunch break
- Weekly visits: Minimum workload of 25 visits per week
- Provider Encounter Notes need to be completed within 48 hours
