The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care to members in a facility setting. As an Advanced Practice Clinician (APC) within SCC, you will provide care to our highest-risk health plan members and be part of an interdisciplinary team dedicated to improving patient outcomes. Primary Responsibilities
- Conduct comprehensive assessments of patients' health status
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD-10 coding
- Communicate and collaborate with the interdisciplinary care team
- Conduct advanced illness and advanced care planning conversations
- Provide patients and caregivers with counseling and education
Required Qualifications
- License: Active and unrestricted NP license in the state of residence, or the ability to obtain by the start date
- Education: NP: Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
- Physical Requirements: Ability to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and various dwelling conditions, and performing physical assessments
- Transportation: Access to reliable transportation
- Collaborative Practice Agreement: Ability to gain a collaborative practice agreement, if applicable in the state
- Vaccination: Full COVID-19 vaccination is required. UnitedHealth Group adheres to all federal, state, and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment
- Drug-Free Workplace: Candidates must pass a drug test before beginning employment
Preferred Qualifications
- 1+ year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
- Understanding of geriatrics and chronic illness
- Proficiency in advanced illness and end-of-life discussions
- Proficient computer skills, including the ability to document medical information using electronic medical records

- Travel Requirements: Daily travel to 2 SNFs in Rochester, NY Monroe County during shift hours
