Qualified candidates will have at least one year of previous experience.You will be providing consultant work at skilled nursing facilities across Bronx and Westchester, New York. Providing follow-up visits following rehab medicine consultations by the physician focused on rehab progress issues including - orthopedic, neurological, amputations, and pain management as a collaborating partner with TeamHealth rehab physician and other facility disciplines. Prior experience with pain management or post acute facilities preferred.Responsibilities:
- Drive high-quality, safe, and cost-effective patient care in post-acute care communities
- Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
