In 2018, Sun River Health merged with Brightpoint Health to create a unified network of care, expanding our reach in New York City and further enhancing the care we offer.Position Overview:Sun River Health is seeking a full-time Nurse Practitioner (NP) to join our team at the St. Ann's Health Center in the Bronx. The NP will work under the direction of a physician to deliver comprehensive, patient-centered healthcare, with an emphasis on preventative care and patient education. The NP will be responsible for planning, evaluating, and providing healthcare services to patients, ensuring the highest standards of care in alignment with Sun River Health's mission.Key Responsibilities: Patient Care: Perform patient assessments, including taking medical histories and conducting physical examinations. Prevention & Education: Emphasize preventative health measures, offering education and counseling on health maintenance and lifestyle changes. Clinical Care: Order and interpret diagnostic tests, prescribe treatments for minor illnesses, and provide follow-up care. Draw blood and perform laboratory tests as necessary. Collaboration: Work closely with physicians and other healthcare providers to ensure high-quality care, referring patients as needed and consulting with other team members. Documentation: Maintain accurate and up-to-date patient records, ensuring appropriate documentation is in compliance with organizational standards and regulatory requirements. Patient Monitoring: Follow up on abnormal results, including abnormal pap smears and reportable STD's, ensuring timely patient care. Case Conferences & Quality Improvement: Participate in case conferences, continuing education programs, provider meetings, and quality improvement initiatives. Infection Control: Adhere to OSHA and CDC guidelines for infection control and safety standards, ensuring a safe clinical environment for staff and patients. Age-Specific Care: Provide care appropriate to the patient's age-specific needs, from infants to geriatrics. Teamwork & Leadership: Collaborate with the multidisciplinary team to meet productivity and quality standards, while maintaining a high level of patient satisfaction.Job Title: Nurse PractitionerLocation: St. Ann's Health Center, Bronx, NYPosition Type: Full-TimeSalary Range: $145,197 - $152,236.50 (based on experience and licensure)Minimum Education & Experience Requirements: Education: Master's degree in nursing from an accredited program. Licensure/Certification: Valid New York State Nurse Practitioner license. Experience: A minimum of 5-8 years of clinical experience, preferably in a large medical practice or community health setting. Preferred Experience: 8+ years of clinical experience, with a strong background in primary care or family medicine. Specialized Skills: Bilingual (English/Spanish) preferred, with proficiency in both oral and written communication. Knowledge and experience in diagnostic and treatment procedures, as well as health education for diverse patient populations. Ability to collaborate effectively with other healthcare providers to ensure continuity and quality of care. Competence in delivering comprehensive healthcare to underserved communities.
