Focus is on providing high quality value-based care in a variety of settings, including patients' homes and various Toledo Clinic primary care offices. Principal Duties & Responsibilities: 1. Assist with covering The Toledo Clinic primary care physician offices on as needed basis 2. Complete Transitional Care Management, Medicare Wellness Visits, or follow up visits for high- risk patients or those who have health issues which prohibit them from getting into their PCP office temporarily. 3. Assist with educating the Toledo Clinic primary care physicians and nurse practitioners in the new economic care models. 4. Help facilitate population health management in a fast-paced environment 5. Attend after hours PCP meetings bimonthly 6. Obtain medical histories. 7. Perform comprehensive medication reconciliation. 8. Identification of risk factors that contribute to rehospitalizations. 9. Diagnose and treat health care and chronic diseases. 10. Order and interpret diagnostic studies such as labs and x-rays. 11. Prescribes medications and other treatments. 12. Promotes positive health behaviors and self-care skills through education and counseling. 13. Collaborate with physicians as needed. 14. Emphasizes wellness and self-care to patients and family members. 15. Researches medical findings and studies and incorporates as appropriate. 16. Selects cost-effective and appropriate diagnostic fees. 17. Other duties as assigned. Performance/Experience Requirements: - Knowledge of nurse practitioner theory, practice and medical care, medical equipment and instruments, and OSHA regulations. - Excellent interpersonal and communication skills. - Ability to analyze and determine solutions in order to best serve the patients. - Demonstrates adaptability to expanded roles. - Consistently arrives at work, in professional attire, on time and completes all tasks within established time frame. - Adheres to Toledo Clinic policies and procedures. - Valid driver's license and current insurance. Education: - Master's degree from an accredited school. - Certified as a nurse practitioner and CPR required.
