What You'll Bring: Unrestricted state licensure as an Advanced Practice Nurse and national board certification. Eligible for Medicare participation. 6+ months of experience as an Advanced Practice Provider (preferably with geriatric, hospice, or palliative care populations). Strong clinical skills and experience in treating chronically ill patients, with a preference for nursing home environments. Tech-savvy with excellent communication skills to collaborate effectively with patients and healthcare teams.
