The Nurse Practitioner (family medicine, pediatric or psychiatry) provides quality health care services within his/her designated specialty, emphasizing the promotion of health and the prevention of disease. The Nurse Practitioner practices independently by collaborating with a designated licensed physician serving as the primary collaborating physician. Individual accountabilities and work volumes are established through the development of annual goals and productivity targets within the framework outlined below.Essential Duties and Responsibilities:* Assesses the primary health care needs, including eliciting comprehensive health histories, performingphysical examinations, ordering and/or performing pertinent diagnostic tests, analyzing collected data to determine patient health status and formulating a problem list.* Provides coordinated and continuous primary health care, pediatric care or psychiatric care (as indicated by training and privileges) to patients that includes mutual goal setting and planning delivery of care with the patient, their family /caregiver and community treatment teams, when applicable.* Prescribes a variety of treatment modalities to meet the health and illness needs of patients.* Documents comprehensive, accurate and continual data in patients' electronic health records within the organizations standards.* Manages and evaluates the plan of care process and outcome and modifies the plan as needed.* Collaborates with other multi-disciplinary professionals and community agencies to provide high quality, age appropriate and culturally-sensitive care.* Coordinates services with other team members, conducts daily huddles and responds to electronic flags to meet patients care needs and to ensure the continuity of care for a panel of up to 1,200 unique patients annually.* Participates in quality assurance activities through collaborative peer review, practice protocol development and identifying indicators for quality care; developing joint practice protocols and identifying appropriate evaluative criteria of self and peers, evaluating clinical standards of care based on the quality assurance process.* Assumes the responsibility for maintaining competence in clinical practice by engaging in a mentor relationship with a designated physician; assumes the responsibility for professional development by regularlyattending HHC Provider meetings, using electronic resources such as Up to Date, developing in depth knowledge of policies and procedures, and participating in continuing education required for clinical licensing and board certifications.* Maintains productivity commensurate with experience, the specific practice setting and annual goals.* Practices within the legal and ethical guidelines established by the American Medical Association, the State of Illinois Department of Professional Regulation (225 ILCS 65/) Nurse Practice Act and Specialty Standards of Practice (e.g., pediatrics, family practice, psychiatry.* Supervises advance practice nursing students when indicated through formal agreements with area universities.Qualifications:* Active Illinois Registered Nurse license. Bachelor's degree in nursing or master's degree or doctorate in advance practice nursing from an accredited institution with a nurse practitioner specialty. Documentation of certification in specialty area must be provided. One to three years of experience as a nurse practitioner desired.* Additional documented clinical experience, education or certification in a related field may be required.* Fluency in Spanish and/or other languages is highly desired.
