McLeod Health Seacoast is seeking a Hospice and Palliative Care Nurse Practitioner. 2 Years Experience Required. Job Duties: 1. Conduct a thorough assessment of patients to understand their medical, physical, emotional and spiritual needs 2. Develop and implement plans to manage symptoms such as pain, nausea, and anxiety, etc. ensuring patient comfort 3. Collaborate with a multidisciplinary team, including attending physicians and case management team to create a comprehensive care plan 4. Provide information to patients and their families about the disease process, treatment options, and palliative principles 5. Offer counseling and emotional support to patients and families, helping them cope with the challenges of a serious illness. 6. Assist patients in making decisions about their care preferences and advance directives.
