The Gerirics Community Living Center Nurse Practitioner provides post-acute and long-term care to adults with have complex medical conditions who reside in our VA nursing home. As part of the Gerirics Extended Care & Rehabilition careline, the provider works as part of an interdisciplinary team and participes in care plan meetings. The Community Living Center Gerirics NP also cares for Veterans receiving the hospice philosophy of care in our Community Living Center. Major duties and responsibilities of the position: Provides leadership in the applicion of the nursing process to client care, organizional processes and/or systems improving outcomes for the CLC and the Gerirics & Extended Care (GEC) service. Obtains medical histories through chart review, telephone histories, face to face s, etc Establishes diagnoses, formuling, and implementing care plans, providing follow-up care. Evalues diagnostic tests to determine the need for further health care or specialty care needs. Orders diagnostic studies and other special tests, such as Magnetic Resonance Imaging (MRI), Computerized Tomography (CT) scans, and ultrasound. Carries out health promotion, disease prevention, and pient educion activities which include reviewing pertinent medical informion with the pient/Next of Kin to include explanion of disease processes, diagnostic studies and trement plans. Orders laborory specimens. Promotes health of the pient and protect from disease. Provides health counseling as needed. Assesses, tres, and determines disposition of residents with emergent conditions consistent with medical staff policy. Orders/consults with ancillary services, such as pharmacy, social work, prosthetic services, etc. Orders VA specialty service consults and initiing requests for non-VA medical care and consults in accordance with VHA and local facility policy and procedures. Writes orders for or prescribing medicions. Completes comprehensive clinical admission History and Physicals assessments within 72 hrs of admission. Completes Periodic Reassessments every 30 days for new admissions (for first 90 days) then every 60 days thereafter. Completes Discharge Summaries as prescribed by facility policy. Participes in daily morning CLC huddles and other meetings as prescribed. Active participant on the Interdisciplinary Trement Team (IDT). Provides educion to pients and their families.
