Responsibilities:
- Provide comprehensive primary care services, including diagnosis, treatment, and follow-up care for patients of all ages.
- Develop individualized care plans and manage chronic conditions such as diabetes, hypertension, and asthma.
- Educate patients and their families on health conditions, preventive measures, medications, and healthy lifestyle choices.
Qualifications:
- Master's or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
- Board certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or other relevant NP certification.
- Experience in a community health, primary care, or FQHC setting.
