Working together with our facility, health system and network partners, Ultra Health is helping to speed recovery and lower rates of hospital readmission.Nurse Practitioner Job DescriptionWorking collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of skilled nursing facility residents. The APRN will establish and build strong working relationships with administrators and staff throughout their designated building and will act as the primary point of contact for emergencies and urgent care visits. Ultra Health provides our APRNs with the training and tools to do more of what you love - working independently to see patients and treat complex medical conditions. Ultra Health offers a highly competitive base salary, plus performance-based bonus.Nurse Practitioner ResponsibilitiesProvide primary, urgent and preventative care for residents in assigned skilled nursing facility. May also provide care to assisted living facility residents.Verify admission orders and conduct initial assessment of residents. Conduct Physical exams, medication review, and depression screeningCollaborate with facility medical director, attending physicians and nursing staff to deliver the highest level of care to the patientCommunicate with and counsel family members on treatment options when there is material changes in statusMinimum QualificationsValid Nurse Practitioner License in GeorgiaDEA LicenseNurse Practitioner national certificationA clinical background in adult, family or geriatric specialtiesVaccination against COVID-19 is required for all new hires unless the individual qualifies for a remote, religious, or medical exemption.Preferred QualificationsExperience in internal medicine, gerontology, or cardiologyExcellent administrative, organizational and verbal skillsAbility to work independentlyDetail orientedDependable and reliableNursing experience in LTC / SNF a plus'$100,000 - $150,000 a year
