We are seeking an experienced Nurse Practitioner to join the Risk Adjustment team, specializing in chronic kidney disease care. As a Nephrology Nurse Practitioner, you will play a crucial role in case management for patients with kidney disease, providing comprehensive assessments, education, and care. Additionally, the NP will collaborate with nephrologists and the care team to complete rounding in the dialysis centers alongside the care team monthly. The main role involves rounding to meet and treat patients and conducting annual wellness visits for the value-based care risk adjustment program KidneyLink, which will take place at the Center or Practice Office. The job may include telehealth (20% of the time) and home visits (10-20% of the time as needed). All travel is reimbursed.
