Key Responsibilities
- Provide high-quality, compassionate care for patients inlong-term care settings, focusing ongeriatric nursing
- Collaborate with nursing staff, medical directors, and specialists to create comprehensivecare planstailored to geriatric patients.
- Accurately document visits in anelectronic medical record (EMR)
- Support acute and chronic care management, ensuring patient needs are met efficiently and effectively as anAdvanced Practice Nurse.
Qualifications
Required Qualifications:
- ActiveVirginia Medical LicenseandDEA License
- Minimum one year of clinical nursing experience
- Strong communication skills, self-motivated, adaptable, and a team player
Preferred Qualifications:
- Nursing experience inICU, Med/Surg, Emergency Medicine, orHospitalistroles
- Experience withcare coordinationand working inlong-term caresettings, especiallygeriatric nursing
