Job DescriptionPURPOSE OF POSITION:This position is being created to have a Nurse Practitioner on a 9:00AM to 5:00PM schedule for Behavioral Health Adult Outpatient Unit to help with the volume of patients and to provide more access to outpatient appointments for our community.
AREAS OF RESPONSIBILITIES:BEHAVIORAL HEALTH OUTPATIENT ADULT
SUMMARY OF DUTIES AND RESPONSIBILITIES:Under a collaborative agreement with an attending Psychiatrist and with the latitude for independent initiative and judgment, the Psychiatric Nurse Practitioner will perform the following functions:1. Provides professional/clinical services to patients in Outpatient Adult Psychiatry to assess and addressthe need for psychiatric and/or substance abuse treatment, psychiatric medication(s),screening and referral for medical care, and follow-up as appropriate.2. Performs a comprehensive psychiatric evaluation that includes assessing current psychiatricsymptoms and past psychiatric history, the psychosocial and physical health status, evaluationof mental status, current and past history of violence, suicidal or self-harm behavior, substanceuse, level of functioning, health behaviors, trauma, and social and developmental history.3. Completes the Outpatient Psychiatric Assessment, Suicide Risk Assessment, AIMS, andappropriaterating scales on the first treatment day with patient.4. Reviews the medical record if the patient has had prior hospitalizations,5. Contacts the patient's outpatient psychiatrist (or clinician) to exchange clinical information ifpatient has been treated outside of hospital.6. Documents any psychiatric, behavioral, substance abuse and active medical problems on theProblem List and Comprehensive Treatment Plan as per policy7. Develops a differential diagnosis derived from the collection and synthesis of assessmentdata.8. Consults with other members of the interdisciplinary team to determine, evaluate, and plantreatment goals/objectives,9. Updates and completes the Treatment Plan Review as needed after the completion of theComprehensive Treatment Plan and/or whenever there is a significant change in the patient'scondition or treatment.10. Records patient's status and progress of care rendered, and plan for future care andtherapeutic interventions appropriate to the patient's needs.11. Counsels and guides patients, their families, and/or significant others relating to physicalhealth, psychiatric treatment, and progress,12. Request appropriate laboratory tests within established protocols, and interprets findings ofrelevant diagnostic and laboratory tests.13. Prescribes and makes adjustments on medication within established protocols, and followsuch medication(s) interventions as appropriate'14. Manages psychiatric emergencies by determining the level of risk and initiating and coordinating effective emergency care.15. Serves as team leader in psychiatric crisis situations.16. Ensures that a patient Behavioral/Safety Plan is in place for all patients, and that teammembers are aware of and consider patient preferences in responding to a crisis.17. Consults with the physician, as appropriate, for evaluation of findings of the psychiatric andhealth screening in order to make prospective decisions about treatment.18. Ensures the timely and appropriate management of a patient's medical problems by communicating with patient's PCP19. Notifies the Team MD and Director of Outpatient Psychiatry of sentinel events, incidents, Code Greys, deviations from standards of cares and problems that may affect the customary and usual activities of the program
Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and2. Current certificate as a Nurse Practitioner issued by the NYSED; and3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

- Bachelors of Science in Nursing (BSN) or Bachelors in allied health discipline = $900
- Masters of Science in Nursing (MSN) or Masters in allied health discipline = $1,200
- Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500
An annual certification differential of $1,878 if you currently hold an appropriate certification that is nationally recognized in the discipline or area of specialization for which you are employed.An annual nursing experience differential ranging from $1,097 up to $9,100 for up to 10 years of experience if you have outside nursing experience as a Nurse Practitioner.
For NP (PC/BH) ONLY: An annual assignment differential ranging from $10,000 up to $25,000 if assigned to a Behavioral Health department
