Overview
Community Health Partners is seeking a full-time Nurse Practitioner or Physician Assistant to join our Urgent Care practice. The APP will join the Urgent Care team to provide quality and timely Urgent Care services to our community. This is a great opportunity to become a contributing member of a team that prides itself in being home to central California's highest level of care.

1. Newly Remodeled Urgent Care clinic located on Herndon between Millbrook & Cedar
2. Urgent Care will be open 7 days a week3. APP will work 12 hour shifts
Accountabilities
1. Provides direct assessment, evaluation and primary patient care, complying with medical protocols and procedures.
2. Provides direct patient management in collaboration with physicians; participates in daily rounds, patient assessments and care plan development.
3. Orders, interprets and evaluates diagnostic tests to identify and assess patient's healthcare needs.
4. Furnishes medications based upon written protocols.
5. Ensures that patients are counseled regarding treatment plans and the use, dosage and possible
side effects of medications.
6. Makes referrals to appropriate providers when patients require subspecialty medical attention beyond the area's scope of service.
7. Documents and maintains precise and complete records of patients.
8. Distinguishes between normal and abnormal findings.9. Provides education, training, information and consultation services to interdisciplinary team, patients, family and caregivers.
10. Participates in quality assurance activities, including assistance in development and implementation of new policies and procedures, recommends updates to current policy as needed.11. Performs other duties as assigned.
Qualifications
Education Completion of a Nurse Practitioner or Physician Assistant program required Credentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures required
Experience 2 years of clinical experience in the area of specialty preferred
Licenses and Certifications If qualifying as a Nurse Practitioner, the following is requiredo NP - Current State of California Nurse Practitioner License requiredo NPF - Nurse Practitioner Furnishing Number requiredo If qualifying as a Physician Assistant, the following is requiredo PA - State of California Physician Assistant License requiredo DEA - DEA Controlled Substance Number requiredo BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) requiredo Ability to participate in a federally funded program (Medicare and Medicaid) requiredo NPC - National Certified Nurse Practitioner by ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) preferredo CRNFA - Certified RN First Assistant preferred
