POSITION DESCRIPTION:

The Family Medicine Nurse Practitioner/Physician Assistant will be responsible for providing primary care services to adults and children. The Nurse Practitioner/Physician Assistant reports to the Medical Director and staff physicians who will serve as the collaborating or supervising physicians.

RESPONSIBILITIES:

  • Conduct comprehensive health assessments of pediatric an adult patients
  • Establish medical diagnoses by thorough history taking and physical examination
  • Order, perform, and evaluate diagnostic studies, prescribe and regulate medications and treatments
  • Manage and follow-up on care plans within the protocols and practice privileges approved by the Westhampton Primary Care Center Medical Director consistent with established policies, procedures and standards of care
  • Document data and findings from physical examinations, diagnostic studies, visits and phone calls comprehensively and according to the standards of Southampton Hospital and the American Academy of Family Physicians
  • Demonstrate an understanding and acceptance of Southampton Hospital policies and procedures
  • Take an active role in the education of patients, students, and staff through information sharing and Case Management meetings
  • Follow infection control practices and procedures for the safety of patients, co-workers and self
  • Cooperate and assist as needed with performance improvement/quality assurance activities
  • Complete the credentialing process and obtain practice privileges at Southampton Hospital as well as participation in various insurance plans

POSITION QUALIFICATIONS:

  • NYS Licensed Nurse Practitioner/NYS Physician Assistant License required
  • National Commission on Certification of Physician Assistants certification required
  • Family Medicine certification preferred
  • At least 3 years of experience in a primary care practice setting
  • Current BLS provider certification from the American Heart Association required

Prior to start date, the selected candidate must meet the following requirements:

  • Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services*
  • Complete electronic reference check with a minimum of three (3) professional references.
  • Successfully complete a 4 panel drug screen*
  • Meet Regulatory Requirements for pre employment screenings.
  • Provide a copy of any required New York State license(s)/certificate(s).

Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.

*The hiring department will be responsible for any fee incurred for examination.
