Physician Assistant or Nurse Practitioner: Saratoga Hospital Center for Breast Care in Saratoga Springs, New York Compensation Range: $114,000 to $141,000
Saratoga Hospital Center for Breast Care, located in award-winning Saratoga Springs, seeks a full-time Nurse Practitioner or Physician Assistant to join our team. The Saratoga Hospital Center for Breast Care works with patients for the prevention, diagnosis and treatment for breast disease. Led by two fellowship-trained breast surgeons, advanced practice providers, and nurse navigators, providers coordinate multidisciplinary care across the continuum of breast care: from new patient assessment, perioperative care, and long-term follow up of patients into survivorship. We seek a team member who is interested in working in a patient-focused environment, with a collaborative practice model. The APP will initiate testing as well as interpret laboratory and imaging data. The perfect candidate for this role is someone with exceptional clinical patient care skills, and an interest in both patient management and patient education. Oncology experience is preferred. Current license to practice as a Nurse Practitioner or Physician Assistant in New York State Saratoga Hospital, which is a Commission on Cancer accredited hospital, has a wide range of services for our patients. We work with an ACR Breast Imaging Center of Excellence. Bi-weekly multidisciplinary breast conference is attended by breast surgery, medical oncology, radiation oncology, pathology, breast imaging and nursing. We have lymphedema therapy, breast rehabilitation, oncology nutrition, and oncology social work support. The Center is open Monday - Friday, and providers have a commitment to a weekend call schedule of approximately 1:5. Hospital rounding on post-surgical patients, as needed, will be required as well as timely responses to urgent follow-up requests. At Saratoga Hospital, our Nurse Practitioners and Physician Assistants serve as an additional patient care provider, augmenting and extending patient services provided by the health care team. The NP/PA will work closely with members of the team and have a reasonable amount of autonomy in the care and medical decision-making for patients. Our general job description of responsibilities: • Obtains, documents and updates medical history. Identifies health risks and needs. Performs physical examinations and conducts preventive screening procedures based on age and history. Formulates a differential diagnosis and plan of care based on history, physical examination, and clinical findings. Identifies needs of the individual, family, or community. • Orders and review diagnostic tests. Identifies, prescribes, and manage pharmacological therapies. • Determines effectiveness of the plan of care through documentation of patient care outcomes. • Reassesses and modifies plan of care as necessary to achieve medical and health goals. • Makes referrals to other health professionals and community agencies. Admits/discharges to appropriate service. • Participates in team meetings and conferences to enhance an interdisciplinary approach to health care delivery. • Serves on departmental and multidisciplinary committees, which influence or determine policies affecting advanced practice providers. • Participates in and develops projects and quality improvement programs, as well as standards of care and practice. • Maintains current level of knowledge relative to professional practice. • Maintains all required professional documentation, such as license, certifications, infection control, ACLS/BLS, DEA as well as system requirements. • Acts as resource to staff and implements educational programs, as needed.
