The Nurse Practitioner/Physician Assistant practices within the context of collaborative management with a physician(s) in diagnosing, managing, and preventing acute and chronic illness and disease, and promoting wellness. Nurse Practitioners have an advanced nursing practice that includes independent nursing functions based on the applicable Scope of Practice, nursing standards of care and a role in medical management within a collaborative agreement with a physician(s).POSITION RESPONSIBILITIES:Assisting the physician in evaluation and management of clinic, rehabilitation unit, skilled nursing, hospital and emergency room patients to include performing histories and physical examinations on new and established patientsScreening patients to determine need for further health care and/or admissionPerforming in office proceduresOrdering diagnostic studies such as laboratory tests, x-rays, and any other studies necessary for the care of patientsEvaluation and management of operative patients to include performing pre-operative testing and counseling as well as post-operative care in clinicIncludes Call expectations, limited to telephone triage and transfersReviews and interprets the medical, surgical and social history of membersInterprets laboratory and other diagnostic data and recommends appropriate treatments or actions for follow-up, reconciles medicationsResponsible for prescribing or performing various treatment modalities - including, but not limited to: medications, injections, wellness counseling, suturing and ancillary testingIs compliant with OSHA, State Board of NursingParticipates in all mandatory compliance and other training programs, seeks guidance for compliance-related concerns and adheres to all applicable laws, regulations, policies and proceduresRefers members to PCP, case management and disease management programs as necessaryPractices in accordance with applicable local, state, and federal laws which govern confidentiality and medical information privacy regulations (HIPAA)Achieves clinical performance targetsParticipates in different roles or interdisciplinary teams as assignedPOSITION REQUIREMENTS:Knowledge, Skills and Abilities:Must be able to demonstrate computer skills, including use of e-mail and Microsoft office word and excelMust be able to demonstrate effective written communication skills including but not limited to writing reports and correspondenceMust be able to demonstrate effective verbal communication skills, including but not limited to speaking effectively to members, providers, families and staff and listening to members, providers and familiesDemonstrated ability to meet deadlines, and use time effectively and efficiently.Demonstrated successful experience working in an environment of change.
