Nurse Practitioner/Physician Assistant - Mobile Unit (req 100584)Albany, NY ( RESPONSIBILITIES:The Community Based Health Nurse Practitioner (NP) or Physician Assistant (PA) will provide primary health care services as a member of a healthcare team comprised of Health Center's physicians, support staff, and other Community Based Health personnel.This position's primary location will be Whitney on Wheels, Whitney Young Health's Mobile Health Unit.SPECIFIC RESPONSIBILITIES:Clinical Responsibilities:Provides primary care directly on a mobile health unit, emphasizing accessibility, health promotion, illness prevention and continuity of care to patients presenting with both minor and complex health issues.Provides primary care at other clinical sites which may include school-based health centers as assigned by the supervisor.Takes a complete history including medical, psychosocial, economic, relationship and family status.Completes physical examinations, performs/orders appropriate diagnostic tests.Manages common acute and chronic illnesses, including minor trauma, episodic problems, and common chronic disorders.Discusses health concerns/diagnosis with patients and families, and together determines an appropriate plan of care.Administers vaccines according to accepted guidelines, and administers medications as appropriate.Provides Medication-Assisted Treatment (MAT), including buprenorphine and Vivitrol.Develops and reviews clinical protocols and best practice guidelines.Quality of Service:Documents all patients encounters completely and in a timely manner. This includes proper documentation of phone calls, prescription, and if applicable, the complete dictation of patient visits for a session prior to leaving the mobile health unit or health centerAdheres to clinical practice guidelines that are adopted by the specialty group or an appropriate committee.Keeps medical knowledge current through attendance at medical CME programs at least 20 hours per year.Maintains an acceptable level of compliance with clinical and regulatory standards including medical record documentation per Joint Commission standards.Demonstrates appropriate interactions toward patients & families in a manner that helps to put patients at ease.Participants in the Performance Improvement activities of the Health Center and Mobile Health Unit through chart audits, development and implementation of practice standardsActively support the implementation of the Chronic Care Model to manage patients with chronic illness.Teamwork:Works collaboratively with the multidisciplinary team to provide treatment, ongoing follow-up and evaluation of the plan of care with the patient.Assists mobile health unit and health center staff willingly in difficult situations in the interest of providing quality service to the patientsDemonstrates constructive problem solving with colleagues and health center staff.Supports established policies and procedures of the Mobile Health Unit. Makes recommendations and contributes to policy formulation.Starts patient schedule on time and leaves after completing the work schedule including needed record documentation.Completes required documentation for referrals and consultations.Returns patient telephone calls within an appropriate time frame based on patient needs and expectations.Performs other administrative tasks as assigned by the Chief Medical OfficerMiscellaneous:Demonstrates excellence in both internal and external customer service.Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.Ensures and/or remains in compliance with local, state, and federal regulations and accrediting standards, i.e. Joint Commission, NCQA, and NYSDOH.Adheres to the National Patient Safety Goals as defined by the Joint Commission and the Whitney M. Young Jr. Health Services.Completes other duties as assigned.TEACHING AND COMMUNITY INVOLVEMENT:Participates in promoting the awareness, enrollment, and utilization of the Mobile Health Unit among community partners and staffParticipants in community activities and research designed to promote health and prevent diseases.Supervises the clinical practice of medical/PA/NP students and residents as appropriate, and contributes to the learning of students from other disciplines.Acts as a resource to staff and the community regarding current trends in health care as well as specific health issues.RequirementsMINIMUM QUALIFICATIONS:Graduate of a registered approved Nurse Practitioner (NP)/Physician Assistant (PA) ProgramCompletion of required State/National Certification ProgramsCurrent licensure and registration in the State of New York, Board Certification as a Nurse Practitioner/Physician Assistant, and DEA license.Current X-Waiver to prescribe buprenorphine and willingness to provide Medication-Assisted Treatment (MAT)Demonstrated proficiency in Microsoft Excel, Windows, Outlook, WordDemonstrated competence in multi-cultural medical settingPrevious experience providing medical care within a pediatric/family medicine practice setting.Basic Life Support (BLS) CertificationPREFERRED QUALIFICATIONS:Two to three years clinical experience preferred.Experience in Community Health, preferably on a Mobile Health Unit.Knowledge of current health care needs and issues of substance use disorder, mental illness, and homelessness.Experience with EMR.Experience in delivering culturally competent healthcare.Interest in on-going learning, best practice development, and education.Bilingual/Multi-lingual abilities.All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability, status as a protected veteran, or any other legally protected status.Salary range: $95,000 - $110,000 annually
