Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Elk River, MN.Our unique, robust model of care goes beyond primary care services — our multidisciplinary care teams collaborate with patients, their families and other healthcare providers to deliver care that is preventative, proactive and tailored to their unique needs.Under our model of care, Bluestone patients experienced 21% fewer ER visits, 36% fewer hospitalizations and 41% fewer hospital readmissions compared to patients with similar conditions and complexities over the same time period.Position Overview:Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communitiesFull-time providers care for patients Monday-Friday with admin time built into scheduleManage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)Ability to design and manage your scheduleDevelop and implement care plans appropriate to patients and their desiresClinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social WorkersAccess to diagnostics such as labs and x-rays as neededCollaborate with community partners within home care, hospice, and other resources as appropriateQualifications:Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education ProgramBoard Certified/Board Prepared through AANP, ANCC, or NCCPALicensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical PracticeAbility to obtain and /or maintain DEA certification for the State of MinnesotaProven ability to communicate effectively with strong verbal skillsExcellent interpersonal and customer service skillsMust have a valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is requiredPreferred Qualifications:2+ years of clinical experience as a Nurse Practitioner or Physician AssistantBluestone providers enjoy:Compensation Base + Quality/Productivity: $115k - 140kGenerous and ongoing CME OfferingUnlimited PTO and Eight (8) observed holidaysGeneral Benefits of:Health Insurance, Dental and Vision InsuranceCompany paid Life InsuranceCompany paid Short and Long-term DisabilityHealth Savings Account (HSA) & Flexible Spending Account (FSA)Retirement plan with 4% matching contributionsAbout Bluestone:Bluestone Physician Services delivers great outcomes by bringing exceptional care to patients living with complex, chronic conditions and disabilities.
