Co-Manage with resident Primary Care Provider (PCP):Provide direct primary care to a panel of continuity patients as if you are the PCP, working independently to manage both chronic diseases as well as acute illnesses wh resident in unavailable due to hospital rotation or vacation.Routine- chronic disease continuity, pre-op eval or employment physicalsUrgent acute complaintsEvaluate a minimum of 6 revisit patients per sessionReview all ordered laboratory, radiographic and ancillary studiesDocument medical evaluation in the Epic Electronic Health Record (EHR)Complete medication refills, patient forms and laboratory re-ordersParticipate in inter-disciplinary care management of high-risk patientsMaintain an evening clinic each week2. Emergency Department (ED) / Inpatient Discharge Visits:Perform focused histories and physicals on patients referred to the APCC from the ED from a recent inpatient admission if they are otherwise unable to get an appointment u their usual resident PCPOrder appropriate laboratory, radiographic and ancillary studies, as indicatedMake referrals to specialists, as indicated, and coordinate future care with the PCPDocument evaluations in Epic EHR3. Triage Between Visit CareHelp coordinators to triage appointment requests in resident firm (i.e. urgency, correct provider, waitlist help)Participate in GIM quality improvement activitiesReview charts/reports for collaborative care and population health managementTypical work week schedule:This is a full-time position totaling 37.5 hours per week. These shifts, whose precise hours are subject to change according to the needs of the hospital, are typically from 8:30AM - 4:30PMMonday through Friday.Minimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and2. Current certificate as a Nurse Practitioner issued by the NYSED; and3.
