Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Job DescriptionGeneral tasks and responsibilities will include:• Provides comprehensive medical and nursing care, including preventive and supportive health care services to patients primarily enrolled in the NYC H+H Community Health Worker Corps program.• Serves as a point of clinical contact and information for Community Health Workers; works closely with the Primary Care Physicians for patients enrolled in the Community Health Worker Program to ensure that clinical care for these patients is met.• Manages health problems and coordinates health care for patients. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.• Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician's intervention.• Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.• Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.• Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.• Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment and other pertinent information. Identifies health and medical risks and needs.• Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.• Orders, conducts, and interprets diagnostic and laboratory tests. Identifies and may prescribe appropriate pharmacological and non-pharmacological interventions and therapies.• Participates in meetings and assists with other clinical and operational tasks related to the H+H Community Health Worker Corps Program.Minimum QualificationsFor Assignment to Nurse Practitioner1. A valid New York State license and current registration to practice as a Registered Professional Nurse; and2. Current certificate as a Nurse Practitioner issued by the New York State Department of Education(NYSED); and3.
