Brief DescriptionJOB DESCRIPTIONFunctional Title: PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONERUnder the direction of the Medical Director, Responsible for providing the health status assessment, maintenance, disease prevention, and supervision of comprehensive health care in collaboration with and under the supervision of the Chief Medical Officer:PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Assess, diagnose, plan, coordinate and evaluate holistic plans of care for BHC behavioral health patients, including treatment with and management of MOUD.
  • Conducts physicals on new participants and current participants in clinic
  • Provides evaluation and treatment of participants during acute illness or emergence of new complaint or symptom; manages participants' chronic illnesses.
  • Determines the need for and facilitates referrals to other medical specialists when participant's condition warrants.
  • Evaluates the participants' plan of care and revises as appropriate.
  • Provides preventive health maintenance for participants including immunizations, screenings, and monitoring of pertinent indicators.
  • Prescribes and adjusts medications within the scope of prescriptive authority and current practice guidelines.
  • Orders, reviews, and provides instruction to participants regarding therapeutic diet.
  • Orders, facilitates, and reviews diagnostic examinations such as laboratory, radiology, etc.
  • Providing psychiatric evaluations, crisis triage, stabilization, and linkage to ongoing outpatient treatment
RequirementsKNOWLEDGE, EDUCATION, SKILLS AND ABILITIES REQUIRED:
  • Master's Degree in Advanced Practice – PMHNP
  • Board Certification through ANCC or AANPCB for Psychiatric–Mental Health Nursing certification
  • Must be a NYS licensed Nurse Practitioner in good standing with all regulatory agencies;
  • Current BLS certification
  • 2-5 years experience in an ambulatory care setting with diverse population;
  • Ability to communicate both orally and in writing in a professional manner;
  • Bilingual Spanish preferred;
  • Excellent interpersonal, oral and written skills;
  • Knowledge of or ability to learn EMR;
