Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations, and nursing standards of care.Examples Of Typical Tasks
  • Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families.
  • Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.
  • Consistent with the established nursing practice, differentiate between normal and abnormal findings in the screening process to detect changes in health status that require physician's intervention.
  • Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
  • Evaluate and interpret patient data, in order to plan and implement appropriate nursing interventions.
  • Consult with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.
  • Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment, and other pertinent information. Identifies health and medical risks and needs.
  • Develop, implement, and evaluate patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.
  • Orders, conducts and interprets diagnostic and laboratory tests. Identifies and may prescribe appropriate pharmacological and non-pharmacological interventions and therapies.
  • Manages a patient session schedule allowing for both scheduled and non-emergent unscheduled visits; maintains a work schedule that ensures availability and continuity of care; ensures optimal availability and utilization of appropriate resources.
  • Provide education, guidance, and direction to nursing staff and other members of the health care team to ensure quality and safe medical and nursing care is provided.
  • Provides education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention, and health promotion, records, and maintains a system of relevant documentation.
  • Performs other related duties, as directed.
Pulmonology DepartmentWe are seeking a dedicated and experienced Nurse Practitioner to join our Pulmonology team in New York City. As a Nurse Practitioner specializing in pulmonology, you will work collaboratively with pulmonologists and other healthcare professionals to provide comprehensive care to patients with respiratory disorders and diseases. You will play a vital role in evaluating, diagnosing, and managing pulmonary conditions, promoting patient education and self-management, and optimizing respiratory health outcomes for individuals across the lifespan.Essential DutiesConduct thorough assessments of patients presenting with respiratory symptoms and conditions, including medical history reviews, physical examinations, and interpretation of diagnostic tests and imaging studies.Collaborate with pulmonologists to develop individualized treatment plans for patients with acute and chronic respiratory diseases, such as asthma, chronic obstructive pulmonary disease (COPD), pneumonia, and interstitial lung disease, incorporating evidence-based guidelines and best practices.Perform pulmonary function testing (PFT), including spirometry, lung volumes, and diffusion capacity, and interpret test results to assess lung function and diagnose respiratory disorders.Provide education and counseling to patients and their families on respiratory health, disease management, medication adherence, inhaler techniques, smoking cessation, and environmental triggers to promote optimal lung function and improve quality of life.Perform and interpret diagnostic procedures and interventions, such as arterial blood gas (ABG) analysis, bronchoscopies, thoracentesis, and pleural biopsies, as indicated for diagnostic and therapeutic purposes.Monitor patients' respiratory status and disease progression over time, adjusting treatment plans as needed to achieve optimal clinical outcomes and prevent exacerbations.Coordinate care across healthcare settings and specialties, facilitating referrals to pulmonology specialists, respiratory therapy programs, and ancillary services as appropriate to address patients' holistic needs.Participate in multidisciplinary team meetings, case conferences, and quality improvement initiatives to enhance the delivery of patient-centered care and promote continuous improvement in clinical practice.Maintain accurate and up-to-date medical records, documenting assessments, interventions, and patient communications in accordance with regulatory and organizational standards.Adhere to ethical and legal standards of nursing practice, respecting patients' rights, confidentiality, and autonomy, and advocating for their best interests.Minimum Qualifications
  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certificate as a Nurse Practitioner issued by the NYSED; and
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
