Job Summary:Provides administrative and clinical support in a professional manner to all nursing staff. Responsibilities may include: providing professional nursing care as determined by the needs of the resident and their plan of care, including but not limited to; dispensing medication, administering treatments, documentation, performing assessments and reporting health status changes, monitoring care given by other members of the nursing team. Participates as a member of the interdisciplinary care team. Demonstrates computer competency as evidenced by documentation using Answers on Demand software for all resident care assessments, care plans and progress notes.Excellent Benefits, including health and ancillary insurances, PTO, on-site childcare, and exceptional educational assistance and employee support programs! 24/7 access to on-site fitness center and indoor pool included!Essential Duties and Responsibilities:•Ensures satisfactory integration of new health center residents by completing a thorough unit orientation with the resident and/or family member. Acts as a support for new residents and their family throughout the moving process.•Provides a customer service model for staff in the delivery of person centered care.•Keeps up to date on New Hampshire state laws and regulations regarding both SNF and Assisted Living care.•Ensures efficient surveys by participating in survey preparation and at the time of survey, provides surveyors with any necessary information needed regarding resident condition and use of MatrixCare.•Displays creative problem solving skills and assures quality of care by direct observation and resident visits to ensure interdisciplinary care plan followed medications and treatments administered in accordance with MD/NP orders.•Oversees schedules, assignments and evaluates staff. Adjusts hours/shifts as needed.•Completes comprehensive assessments of residents as needed to monitor health status, acute changes and appropriate care interventions.•Assists independent living residents with urgent health care needs by responding to pendant alarms or emergency calls in a timely manner; assesses the resident's condition; contacts the medical provider or arranges for hospital transfer as appropriate.•Audits care plans for accuracy prior to plan of care.•Completes annual performance reviews for direct report staff within assigned timeframe.•Utilizes team-building skills through by coaching and counseling, and assisting with the orientation of new staff as needed.•Maintains ongoing communication regarding nursing staffing needs, clinical updates, and critical incidents with the Director of Nursing.•Supervises Nursing staff including checking that documentation has been accurately completed, that assignments are carried our and appropriate care is being rendered.
