BASIS FOR AWARD WILL BE LOW PRICEJob Description: • This is a professional nursing position which involves performing assessments of chronically ill Medicaid clients to receive/maintain home care services and reside safely in the community. Assessments include reviewing the medical doctor's orders, conducting client visits, completing the Uniform Assessment System for New York (UAS-NY), developing care plans, utilizing a time task tool, and creating service orders. RN staff will document in the client's case file his or her medical diagnosis, suitable care plan, and determination of the daily/weekly hours of home care services required and a justification of same. General Requirements :The Selected Respondent shall provide the following: 1.(a) At least four (4) Full Time Equivalents (FTEs) who are Registered Nurses (RN's) with: a) home visiting experience; b) a current NYS RN license; c) a current valid NYS driver license; and d) either a bachelor's degree in nursing from an accredited college or university, or an RN license with at least two years of recent home care experience.2. Recruitment of nurses and assistance in the interview process. 3. Reimbursement for daily mileage incurred for contracted home visitation, but not for transportation to and from home/workplace. 4. Retention of all necessary professional licenses or registrations for its employees during the term of the resulting Agreement. Upon request, the selected Respondent will provide MCDHS with copies of these documents. 5. Pre-requisite background checks on all personnel, to the extent now or hereafter permitted by law, including fingerprinting and/or drug testing. 6. Maintenance of staff health and development files, including CPR certification and other required certifications. Health files will include an annual health assessment consisting of a pre-health assessment (including pre-employment drug screen), Rubella vaccine and annual PPD for staff. Development files include documentation of professional licenses, renewals, and related materials. 7. Ensure that all work performed is in accordance with federal, state and local laws and standards. 8. Compliance with all statutory/regulatory/local requirements for timeliness and quality of assessments.
