This position involves assisting with daily living activities, maintaining a safe and healthy environment, and working collaboratively with nursing staff to ensure quality patient care.Responsibilities:Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding.Help residents with mobility, transfers, and positioning to enhance comfort and prevent falls.Measure and record vital signs, including temperature, blood pressure, pulse, and respiratory rate, and report any changes to nursing staff.Maintain accurate records of patient care activities, changes in condition, and any relevant observations in the electronic health record (EHR) system.Ensure cleanliness and safety of resident rooms, communal areas, and equipment. Adhere to infection control and safety protocols.Provide companionship and emotional support to residents, promoting their dignity and independence.Communicate effectively with residents, families, and the healthcare team regarding residents' needs and concerns.Work collaboratively with nursing staff, physicians, and other healthcare professionals to deliver coordinated care.Required Minimum Education:High school diploma or equivalent is required.Licensure/Certification:Current certification as a Certified Nursing Assistant (CNA) in New York State is required.Basic Life Support (BLS) certification is preferred.Required Minimum Experience:Previous experience in a skilled nursing facility (SNF) or long-term care setting is preferred.Experience with Point Click Care (or similar electronic health record systems) is beneficial.
