Current Employees: If you are currently employed at SOMC please log into UKG Pro to use the internal application process.Department: HospiceShift/Schedule: Part Time (24 hrs/wk), Day ShiftGENERAL SUMMARYWorks under the supervision of the Outpatient Nurse Manager. The Hospice Aide's primary job function is to provide personal hygiene, supportive care, and light housekeeping for Hospice patients and families. This position assists the caregivers in maintaining the patient in a home setting by helping with day-to-day needs on an intermittent basis. Performs other duties as assigned.QUALIFICATIONSEducation:•High School Diploma or successful completion of an equivalent High School Exam Required•Completion of approved nursing assistant, home care aide, or long term care nursing assistant training or equivalent experience required•Home Care competency evaluation program within 90 days of hire requiredLicensure:•BLS within 90 days of hire required•BMVL required (Must maintain eligibility to drive as determined by SOMC insurance carriers)Experience:•Approximately six months prior experience in either the acute, long term care or community based care setting preferred. Home Care experience preferred.Interpersonal Skills:•Necessary in order to interact with patients, nurses and physicians in a courteous, respectful manner.JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONSThe following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time. 1. Makes home visits, home-like setting visits, and residential care facility visits and provides care, comfort, and safety of assigned patients under the direction of the Primary Care Nurse.2. Performs basic personal care activities i.e., complete and partial baths, assists and instructs patients/families in oral hygiene and denture care, perineal care and skin care, and position patients, etc.3. Performs basic patient care procedures i.e., giving enemas, applying non-sterile dressings, assists patients/families in colostomy care, collecting routine specimens, and performs post mortem care.4. Assists patient/family with light housekeeping activities in patient environment only. Prepares or assists with light meal preparation for patients.5. Reports significant physical/mental patient changes to RN.6. Helps patient achieve quality of life through providing companionship and emotional support. Provides reassurance, listening, and encouraging social contact.7. Completes clinical record documentation for every visit per policy and procedure.8. Participates in in-services, department meetings, and workshops to advance knowledge and skill.9. Assists with patient/family coverage by showing a willingness to be flexible to meet patient/family preferences/needs as well as department needs.Thank you for your interest in Southern Ohio Medical Center.
