Job Summary
Provides direct care as assigned and works in the center. Responsible for the collection of data related to participant status; performance of basic nursing procedures; ongoing collection and documentation of participant data; assistance with treatments and procedures; and assistance with maintenance of patient care equipment. May require access to storage areas where medications are kept and carry out procedures that require the use of non-prescription medications.
Essential Functions

* Provides participant care based upon individualized plan of care as directed by the Interdisciplinary Team, including the participants' physical, emotional needs, cultural, spiritual, psychosocial, and educational needs.
* Documents and reports patient status and condition. Maintains and ensures accurate maintenance of participants' medical records and treats records as confidential.
* Demonstrates effective communication skills in collaboration with other Interdisciplinary Team members to ensure optimal functioning of the center.
* Identifies opportunities for process improvement and participates in on-going process improvement activities.
* Performs other duties as assigned.
Supervisory/Management Responsibilities
* This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
* Education - High School Diploma or equivalent - completion of a Nursing Assistant or Patient Care Tech program
* Experience - One (1) year of experience working with a frail elderly population
In Lieu Of
* In lieu of school verification of a nursing assistant training course, may accept current or former South Carolina CN A certification as proof of completed state required training at time of hire.
