Job SummaryUnder the direct supervision of the LPN and/or Director of Nursing, the Nursing Assistant provides a variety ofdirect resident care activities in order to provide optimal resident comfort, safety, companionship, cleanliness and dignity.Interacts with residents in a variety of ways and settings, including social, recreational, diversional and/or vocational ways.ResponsibilitiesActivities of Daily LivingAssists with meals, including serving, clearing and escorting residents to and from dining room. Ambulates residents as directed. Receives report for changes in resident conditions. Provides baths/oral hygiene on a daily basis.Provides skin care according to established protocol. Takes opportunities to educate patients while performing ADLS (i.e. inspirometer, provides instructionon walker when ambulating patients). Interprets and communicates patient care data, observations and information to other health careteam members. Ensures resident commons areas and private living accommodations are safe, clean, tidy andcomfortable. Oversees residents with Alzheimer's and other dementias and focusing on one or more dimensions ofhealth and wellness. Provides companionship for residents (activities, puzzles, etc.) focusing one or more. Completes laundry services for all residents including washing/drying/steaming, ifneeded/folding/hanging items and returning items to the apartment. Performs daily basic cleaning of apartment including bed making, organizing, closets, taking outtrash, washing dishes, etc. Provides support in absence of other services (i.e. cleans carpets, unclogs toilets, assists with diningservices, and escorts resident to a physician's appointment or Emergency Department).Clinical CareProvides resident care safely in a calm, efficient and professional according to MHS policies,procedures and established practices. Always responds appropriately to the emergency call system alerts as per established procedures. Performs all required interventions and/or procedures as directed by LPN/Director. Demonstrates teamwork by communicating to other staff via shift report and other communication. Effectively prioritizes tasks so that resident(s) is ready for daily activities. Interact with residents, always focusing on the personal needs of the resident. Obtains vital signs and reports any abnormalities. Always is thorough in completing tasks, assuring everything is returned within the patients reach (i.e.call bell, phone, water pitcher, making hourly rounds). Obtains vital signs and blood sugars and knows when to question results and notify the RN ofabnormalities. Answers call bells in a timely manner. Initiates comfort measures to relieve pain within scope of care. Consistently reports resident complaint of pain to the LPN/ Director of Residential Care. Interprets and communicates patient care data, observations and information to other health careteam members including all changes in patient conditions, activities and age-specific care. Provides reminders for meal assistance, assists with menu choices, set up, documents intake of foodand fluids. Distributes nourishments/snacks as requested/ordered. Ambulates resident per physician orders, care plan and/or staff nurse instruction. Performs all required interventions and/or procedures to provide for total patient care in the clinicalsetting. Performs incontinence care and assistance as needed. Knowledgeable about chronic physical and mental conditions affecting old adults and providedappropriate person centered care.Documentation and Regulatory ComplianceAccurately and timely enters vital signs, height, weight and ADLs, on resident chart. Efficiently completes proper documentation in the resident record and weekly activity log within workshift. Signs all entries. Recognizes signs, symptoms and instances of abuse and neglect and demonstrates knowledge ofmandatory reporting requirements within scope of responsibility.Equipment UsageOperates equipment accurately and safely, according to policy, procedure and or manufacturer'sdirections. Ensures equipment is prepared for next procedure and stocks supplies. Tags and reports malfunctioning equipment in a timely manner to Director of Residential Care. Quickly learns to use new equipment and can train new staff on its use. Uses equipmentindependently. Demonstrates knowledge of basic electrical safety.Performs Administrative Duties for UnitInitiates comfort measures to relieve pain within scope of care. Consistently reports patient complaint of pain to the RN.Performs Other Duties as RequiredDemonstrates the ability to perform duties in a safe, calm, efficient and professional manner. Consistently demonstrates ability to diffuse situations. Functions efficiently and professionally as a team member and takes the initiative to assist teammembers in completing assignments through prioritization of resident tasks. Attends a minimum of 4 staff meetings per year. Maintains job requirements, attends mandatory in-services, actively participates in self-education andcompletes all required competency assessments on time. Performs other duties as required.Knowledge, Skills & AbilitiesHeavy Work: Frequent lifting and/or carrying of objects weighing up to 50 lbs with a maximum lifting of 100 lbs.Demonstrated proficiency in assisting dependent population withActivities of Daily Living and Vital Sign monitoring experience preferred. Knowledge of long-term care population practicesand procedures and/or one year prior nursing assistance experience preferred.
