He/She carries out simple treatment procedures and follows the instructions of the charge nurse. He/She also works closely with the activities department, volunteers, housekeeping, and all other ancillary departments. He/She has contact with members of resident's families. **This position qualifies for a $1,000 sign-on bonus**Qualifications: The State of Minnesota requires all nursing assistant/registered must have satisfactorily completed a Minnesota approved competency evaluation program of at least 75 hours. At completion of the 75 hour program and successful completion of the final test, the nurse assistant will apply for placement on the Minnesota Board of Nursing Registry. The nursing assistant/registered is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.General Responsibilities:
- Assist the resident with hands on care for all ADL's as contained in the resident plan of care. This includes dressing and undressing, grooming, mouth care, toileting, bathing, showering, & walking. He/She periodically checks on the resident and positions and turns the resident as indicated in the resident care plan. The Na/r also responds to resident's call lights, and pushes wheelchairs, geriatric chairs, and performs other transferring techniques.
- Assist residents with eating and drinking according the resident's plan of care. He/She offers water, juice and/or other nourishment to resident according to schedule and whenever needed.
- Observe the resident's skin condition and assists with treatments described in plan of care. He/She cleans skin including perineal area. He/She observes for and reports any open wounds, rashes, and any other break in skin integrity.
- Assist the residents with toileting, commode transfer, catheter care, emptying and cleaning colostomy bags and reports output.
- Take and record vital signs including blood pressure, temperature, pulse, and respiration. He/She observes abnormal changes in body functioning and/or behavior (eating habits, skin condition, mental status, mobility, etc.) and alerts the charge nurse.
- Read and understand the plan of care for each resident on daily basis. He/She follows all instructions as contained in the plan of care. He/She follows all rehabilitation procedures, such as those describing lifting, transferring, ambulating, and positioning of the resident's.
- Provide needed assistance and encouragement in getting resident to and from activities, events, meetings, etc. that provide opportunities for socialization with other residents. He/She promotes resident's right to make personal choices to accommodate his/her needs. The Na/r promotes the resident's right to be free from abuse, mistreatment, and neglect and reports to appropriate facility staff any instance of such treatment. He/She offers support and comfort to resident who is dying and his/her family. He/She talks with and listens to resident during cares.
- Attend a minimum of 12 hours of in-service education annually as set forth by the Minnesota Board of Nursing.
- Participate and assist with resident evacuation procedures in case of fire, disaster or other occurrences requiring the resident be moved. He/She practices proper body mechanics and incorporates safe practices into all job duties. He/She provides orientation to new and/or current employees as directed. He/She assists in special projects as needed by the Director of Nursing. He/She practices infection control procedures. He/She meets attendance expectations.
- Actively participate in and implement quality performance in all aspects of job responsibilities and functions. This includes all interactions with residents, staff and visitors. Participates in and promotes teamwork. He/She seeks, recommends, and/or implements new methods and means of providing quality service and continuous improvement.
- Basic competencies of abuse, neglect, exploitation, and misappropriation of resident property.
- Basic competencies of dementia management, infection control and other areas as identified through the facility assessment.
- Knowledge, skills and abilities required by OSHA Hazard communication, HIPPA, QAPI and other organizations. Respond to emergency codes, ie: fire
- Knowledge, skills and abilities in Fall Prevention, Operation of exit alarms, Transmission-based precautions, Identification and reporting changes in resident's conditions, Job specific competencies.
- Knowledge, skills and abilities in Person centered care, Communication, Basic nursing skills, Basic restorative services, Skin and wound care, Medication management, Pain management, Additional infection confection control topics, Cultural competency
- Skilled in Communication - Verbal and Written
- Specific care skills as identified through the Facility Assessment
- Follow and abide by the HIPAA (health insurance portability and accountability act) privacy as administered by Heartland Senior Living.
