Job Description:

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.We are seeking a Full-Time Nurse Case Manager in King of Prussia, PA to deliver care to individuals with disabilities in a home health setting, utilizing the nursing process of assessment, planning intervention, implementation, and evaluation.Starting rate is $34.00/ per hour ResponsibilitiesParticipate in the admission of new residents to assigned homesDevelop plans to guarantee that the medical and healthcare needs of the residents are met and ensures the appropriate medical follow-upReview lab work and coordinates annual physical exams and care with clinics, outside physicians, and hospitalsProvide special instructions to resident and home staff related to medical diagnosis, allergies, diet, etc., and is available for ongoing consultation to staff regarding medical problems and healthcareClarify medication orders and contact physician to review consumer's status and medication plansParticipate in quarterly and Annual Individual Plan (IP) meetings for assigned participantsCreate and distribute Individual Plan Medical/Health Reports detailing participants' general health, health care goals, and physician recommendations for each participantProvide documentation regarding individual's health status for various support peopleMake referrals for needed medical services and ensure that follow-up care is pursuedProvide quarterly in-service training for support staffAdvises supervisor of all concerns and/or issues related to the provision of nursing servicesEmployees must cooperate with the licensee and department staff in any inspection, inquiry or investigationFollows strict OSHA procedures regarding blood borne pathogensMinimum RequirementsCurrent Nursing License requiredExperience with Chapter 6400 regulationsMinimum 2 years' experience as an RNBachelor's degree preferredDiabetes Education Certification preferredAbility to calculate figures related to medical dosagesAbility to establish a comfortable and supportive relationship with a person receiving supportsValid driver's licenseWhy Community Options?
