Overview:

The LPN Nursing Coordinator supports quality, patient centered care. The LPN performs a variety of high-level functions within a care team to support the needs of the healthcare team and patients/support network. The Nursing Coordinator, LPN coordinates the management of the care through a well-defined care delivery system.
Responsibilities/Job Description:
- This position would require strong communication skills and relationship building with referral sources
- Promote safe and effective patient and organizational environments and feeding equipment use
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.
- Respond in a timely manner to questions, concerns or emergency situations from staff, patients, and referral sources.
- Interact with referral sources to facilitate communications, answer questions, and resolve problems. Answer and triage incoming calls, referring all clinical assessment and intervention to the Clinical Team.
- Responsible for ensuring patients receive outstanding service, education, and resolution regarding Enteral nutrition by promoting supplies and providing necessary education to gain/maintain optimal clinical outcomes. Contacts patients and caregivers for enteral formula and supply needs and maintains computer schedule for reorder dates. Coordinates delivery of enteral formula and supplies to patients.
- Provides patient and community education regarding all aspects of home infusion and enteral nutrition services provided. Coordinates initial delivery of enteral formula and supplies to patients.
- Schedule nursing visits for FHI nursing staff following established productivity standards, clinical expertise, and staffing availability. Coordinator is responsible to coordinate and maintain scheduling for patients and staff. Answer customer inquiries regarding the status of their visits, orders or supplies and providing excellent customer service.

- Computer literacy
- Knowledge of infusion and enteral therapy-- access devices and administration systems.
- Serve as a primary point of contact for enteral patients and their families, addressing inquiries and concerns with empathy and professionalism.
- Provide ongoing support and follow-up to ensure patient satisfaction and adherence to enteral nutrition therapy.
- Manage inventory and ensure the timely ordering and delivery of enteral nutrition supplies and equipment.
- Maintain accurate records of supplies, ensuring proper documentation and compliance with regulatory requirements.
- Coordinate with suppliers and vendors to resolve any issues related to supply availability or quality.
Qualifications:
Required
Education
Completion of a certified LPN program.
Experience
Minimum of two years recent experience in home care, long term care, clinic, urgent care or med-surg/acute care setting
License/Certification/Registration
Current LPN license in the State of Minnesota.
Basic Life Support (BLS)

Experience
Minimum 2 years recent experience in home infusion therapy including direct patient care.
Previous customer service experience preferred
