Job Description:
SUMMARY:The provider collaborates under the direction of the Medical Director in resident care management to include assessments, care planning, and evaluation and implementation of appropriate treatment plans.ROLE AND RESPONSIBILITIES:
  • Acts within the scope of practice as defined by state law, as outlined in Section 64.5 of Title 8 (education) and parts 94 and 415 of New York Codes Rules and Regulations.
  • Performs admission assessments and scheduled resident visits, alternating with the Medical Director or attending Physician after the initial admission to the facility.
  • Participates in regular rounds according to a mutually agreed upon schedule meeting New York State Department of Health regulations. May also perform acute resident visits as deemed necessary.
  • Participate in facility audits as time permits.
  • Assesses resident information and clinical data identifying both normal and abnormal findings. In conjunction with the clinical findings, it is the expectation that the Nurse Practitioner will identify and order appropriate routine tests and procedures deemed necessary based upon clinical practice guidelines and the overall plan for the resident.
  • Reviews and signs appropriate medical orders or verbal orders
  • Reviews periodic Occupational and Physical and Speech Therapy evaluations.
  • Completes discharge orders and prescriptions. Responsible for signing the discharge summary.
  • Completes pre-employment physicals on all newly hired staff of Heritage Ministries
  • Interacts courteously with all residents, family and general public. Functions as a liaison between physicians, residents, staff, and family members.
  • Maintains strict confidentiality of information as it relates to residents and the business of Heritage Ministries.
  • Contributes to Continuous Quality Improvement program. Assists in development of goals and approaches to resolve and prevent problems.
  • Observes and promotes resident's rights at all times.
  • Any additional duties as assigned by management.
RequirementsQUALIFICATIONS AND EDUCATION REQUIREMENTS:
  • Two to five years of experience in a related position preferred.
  • Current Licensure
  • Graduate of an accredited program
PREFERRED SKILLS:
  • Excellent verbal and written communication skills
  • Strong analytical and problem-solving skills
  • Experience with Electronic Medical Records strongly preferred
PHYSICAL REQUIREMENTS/WORKING CONDITIONSThe physical requirements and working environment are typical of most medical office settings. Employees are frequently required to sit for long periods of time, stand, walk, balance, hear, talk, reach, and use their hands. Employees may occasionally be required to bend, stoop, kneel, crouch, climb (stepstool) and stretch. Physical effort requires some lifting usually not in excess of 15 pounds. The office work environment is indoors and free from dust, smoke, moisture, and excessive noise.
