- The Nutritionist provides on-demand diabetes education for walk-in patients and collaborate with internal medicine and family medicine teams for seamless care.
- Maintain accurate and thorough clinical documentation in alignment with established standards and guidelines.
- Collaborate with an interdisciplinary diabetes education team, including certified educators, pharmacists, nurses, and technicians, to optimize patient outcomes.
- Partner with outpatient clinic teams to deliver continuity of care, addressing same-day patients.
- The Nutritionist performs both outpatient and inpatient public health nutrition services, including individual consultations and group education sessions.
Nutritionist - REQUIREMENTS
- Certification as a Certified Diabetes Care and Education Specialist (CDCES) is preferred.
- Minimum two years of experience in diabetes education for all ages, including gestational diabetes.
- Proof of flu and COVID-19 vaccinations.
- Current Registered Dietitian license in any state.
