- Establish strategies and best practices for utilizing Occupational Therapy techniques with unhoused and recently housed populations.
- Provide interventions as part of a multidisciplinary team, including conducting assessments to screen for limitations, recommending energy conservation techniques, durable medical equipment, and strength-building exercises.
- Make referrals to appropriate community-based services, offer psychoeducation, and conduct apartment assessments to recommend modifications as needed.
- Provide professional consultation to street/subway outreach team staff to support services for older adults and medically fragile clients.
- Participate in multidisciplinary meetings.
QualificationsRequired Knowledge, Skills, and Abilities:
- Candidates must have a Master's Degree in Occupational Therapy as well as current valid NYS licensure, or temporary licensures at the time of hire.
- 3 or more years of experience working with older adults, individuals with chronic medical conditions, people precariously housed or unhoused, and/or related fields.
- Ability to travel throughout New York City.
- Computer literacy is required. Ability to use agency Electronic Health Records to document services and track data.
- Bilingual in Spanish is a plus.
