* Initial Evaluation pf patients functional status in a home setting
* Development and implementation of a plan of care
* Ongoing one-on-one treatment in the home
* Educate patients and caregivers on the plan of care and individualized treatment plan
* Maintains and submits all clinical documentation in compliance with Agency standards
**Requirements:**
* Occupational Therapy License New York State
* COVID vaccination required
* Minimum one year clinical practice. Home Care preferred
* Valid NYS Drivers License and reliable transportation
* Travel throughout the Borough of Queens
