Therapists will ensure that they communicate, educate, and advocate while performing therapy in a fiscally responsible manner.*Duties:** Evaluates, plans, and provides occupational therapy treatments for hospice patients as prescribed by the physician.* Provides patient/family, RN Case Manager, and LPN both verbal and written instructions in occupational therapy techniques and equipment as ordered by physician, including demonstration.* Implements the following services in the home:Adaptive work skillsOrthoticsPractice in functional skills and activities related to daily living* Consults via phone or home visit, with RN Case Manager to evaluate patient progress, if needed.* Completes documentation of all interventions, evaluations, and observations in the hospice clinical record in a timely manner when services are provided to a hospice patient.*Skills/Qualifications:*Qualified candidates will possess a current and valid New York State Occupational Therapy license, preferably with one year experience.
