Occupational Therapist Location: Rushville, ILDuration: Fulltime Shift Information: DaysDescription: Required Certifications: Registered with American Occupational Therapy Association, Graduate from approved school of Occupational Therapy, IL State Licensure, Current BCLS cert. Responsible for the evaluation, planning, directing and administering of occupational therapy modalities of treatment as prescribed by a licensed physician. Administers treatments and occupational agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities. The staff therapist coordinates, delegates and supervises responsibilities assigned to supportive staff (OTA, Patient Care Assist., etc.), and participates in scheduling patients, as well as all other aspects of patient care within the Department. The OT participates in operational aspects of the department, maintains performance improvement activities within the department and participates in QA activities. Participates in all infection control, departmental equipment training, organizational safety and fire safety programs.Duties And Responsibilities: E = Exceeds the Standard M = Meets the Standard NI = Needs ImprovementDemonstrates Competency in the Following Areas: E M NI Delivery of Patient Care and Application of Knowledge Performs treatments and provides services to level of licensure and other duties as assigned within the LPN scope of practice. 2 1 0Able to provide detailed and timely patient report for the continuation of care and planning at shift change and upon handoff of care 2 1 0Demonstrates ability, under the supervision of an RN, to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes pediatric, geriatric and the general patient population. 2 1 0Evaluates, updates and revises individualized plan of care for each patient based on patient diagnosis and condition, individualized patient and family learning needs, and progressive goals for an effective transition of care and disposition 2 1 0Knowledgeable the physiological basis for care delivery based on patient condition and/or diagnosis and institutes nursing actions based on the medical plan of care and accurate patient assessment 2 1 0Notifies RN and/or provider timely of changes in patient condition and/or abnormal test results 2 1 0Knowledgeable of common lab and diagnostic test results and able to relate abnormalities to condition and care delivery 2 1 0Demonstrates proficiency in the initiation and/or management of peripherally IV's and central lines. 2 1 0Completes accurate and timely documentation of patient care status, medication administration, and clinical care activities 2 1 0Able to organize patient care priorities and delegate tasks based on competency and licensure 2 1 0Effectively communicates all procedures, treatments and medications to the patient and/or family 2 1 0Provides for a safe patient environment to include call bell and personal belongings proximity, bed rail and bed position, clutter-free area, and frequent patient rounding for care needs 2 1 0Ensures correct patient identification before any medication or treatment delivery by confirming the patient's identification band and utilizing two patient identifiers 2 1 0Includes patient and/or family as part of the care team 2 1 0Medication Administration: Administers medications safely and timely ensuring the rights of medication administration are followed 2 1 0Demonstrates knowledge of medication purpose, dosages, side effects and contraindications 2 1 0Safely administers, monitors and documents blood and blood components per policy 2 1 0Able to recognize and take the appropriate actions for any medication or blood/blood component reaction 2 1 0Professional and Personal Responsibility: Adheres to the principles of good body mechanics when lifting and positioning patients 2 1 0Completes incident reports timely to document any unusual occurrences 2 1 0Reports to work on time and able to complete patient care activities within work shift without compromising quality 2 1 0Adapts to changing workload as required 2 1 0Assists in the orientation and integration of new employees 2 1 0Recommends process improvement initiatives to improve patient care, quality and safety, and productivity 2 1 0Knowledgeable of role in emergency/disaster plans 2 1 0Complies with all isolation and infection control practices 2 1 0Maintains all required certifications, licensure renewal and CEU's, and completes mandatory education 2 1 0Maintains knowledge of and adheres to all hospital policies 2 1 0Complies with all requirements of the Illinois Department of Public Health Licensing Act, Illinois Nurse Practice Act, OSHA, CMS and other regulatory and professional standards and hospital policies 2 1 0Adheres to dress code, appearance is neat and clean 2 1 0Wears identification while on duty, uses computerized punch time system correctly. 2 1 0Interpersonal Skills / Communication / Human Relations: Communicates in a courteous and respectful manner with patients, families, visitors, medical staff, nursing personnel and other departments 2 1 0Attends staff meetings regularly and collaboratively interacts to generate ideas and outcomes 2 1 0Maintains ethical standards related to confidentiality, HIP nd DHHS privacy and security regulations 2 1 0Represents the organization in a positive and professional manner. 2 1 0Promotes and actively participates in the C.A.T.C.H. initiative 2 1 0Maintains CMH Values (CARES): Collaborative Communication - that is transparent, respectful and maintains integrity 2 1 0Accountability - takes ownership of actions, learning from mistakes and continuously improving 2 1 0Respect - exhibited in our attitudes, communication and actions 2 1 0Ethical Behavior - based on honesty and integrity that drives all other values 2 1 0Service - to the community, the providers and each other 2 1 0Education and Experience Requirements: Graduate of an accredited nursing program;
