Outreach Rehab is growing in-home outpatient physical, occupational, and speech services in New York and New Jersey. Due to our continued growth, Outreach is looking for motivated and passionate Therapists to work with the adult and geriatric populations in private homes, apartments, or Assisted Living Facilities. Position Summary:
In this position, you will perform in-home Occupational Therapy.
Responsibilities:
- Home safety assessment: Assess environmental barriers and extrinsic risk factors for falls in the individual's environment.
- Ordering adaptive equipment.
- Conducting cognitive and/or visual assessments.
- Addressing underlying impairments related to ADLs (Activities of Daily Living) and IADLs (Instrumental Activities of Daily Living).
- Educating patients on home programs along with caregivers/family members.
Job Qualifications:
- Master's degree (minimum) in Occupational Therapy (OT).
- 1 year of experience working with older adults preferred.
- Home care experience is a plus.

Compensation:
- Full-time: $89,000.00 - $104,000.00 per year
- Per diem or Part-Time: $60-$74 per hour
