Job Description:
Shift Schedule: Monday-Friday Hours Per Week: 10.5DUTIES AND RESPONSIBILITIES:Under the supervision of the Program Director, the Occupational Therapist collaborates with other program staff to assist with completing needs assessments, most specifically those focused on the participant's activities of daily living/instrumental activities of daily living (IADL/ADL). This role also assists with determining needed fall prevention aids and identification of specific referrals necessary to maintain safety at home. Job responsibilities will include:
  • Conduct ADL and/or IADL assessments, develop and maintain service plans, make referrals as deemed appropriate, and facilitate successful service connections.
  • Assisting clients of Living In Place to develop or maintain adaptive skills, and/or performance abilities designed to achieve maximal physical, cognitive and mental functioning of the client associated with his or her activities of daily living and daily life tasks.
  • Review discharge plans for clients who have been recently released from hospital settings.
  • Provision of time-limited care management service and targeted interventions and monitoring of progress in compliance with the Living in Place program standards and timelines; services to be provided in home, office, and other community settings.
  • Referral and cooperative relationships with other community agencies, to meet the needs of clients.
  • Ongoing reassessment of client ADL and/or IADL needs, monitoring of service efficacy, and appropriate revision of the service plan.
  • Informing supervisors of situations of crisis or circumstances that require further guidance.
  • Represents and interprets the agency at community conferences and meetings.
  • Participates in training and staff meetings.
  • Attend training as assigned in specific evidence-based treatments and as required by DOHMH and JASA.
  • Participate in training and unit meetings.
  • Completes required case records, reports, and statistics in compliance with JASA and DoHMH standards.
  • Performs such other duties as assigned.
QUALIFICATIONS:
  • Bachelors Degree in Occupational Therapy, or
  • A post-baccalaureate certificate in occupational therapy from an institution acceptable to the NYS Department of Education.
  • NYS licensure in Occupational Therapy required.
  • Good writing/record-keeping skills.
  • Ability to read, write, speak, and understand English. Ability to speak and understand Spanish is preferred.
  • Must be computer literate, including the ability to learn and use program-specific software.
  • Ability to travel to clients' homes and to negotiate any steps and barriers in clients' residences.
  • Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.
