Job Summary We are seeking a dedicated and compassionate Occupational Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating and treating patients with various physical, emotional, and developmental challenges. This role requires a strong understanding of occupational health practices and the ability to work effectively with individuals across different settings, including inpatient, outpatient, and home health environments. The Occupational Therapist will play a vital role in helping patients achieve their rehabilitation goals and improve their quality of life. Duties Conduct comprehensive assessments of patients to identify their needs and develop individualized treatment plans. Implement therapeutic interventions aimed at improving patients' daily living skills and overall functional abilities. Collaborate with other healthcare professionals, including physical therapists and speech therapists and unit coordinators, to ensure a team approach to patient care. Provide education and support to patients and their families regarding treatment plans and progress. Document patient progress accurately in accordance with medical terminology standards. Participate in discharge planning to facilitate smooth transitions for patients moving from inpatient care to home health or outpatient services. Maintain up-to-date knowledge of best practices in occupational therapy. Oversees the Certified Occupational therapist assistants to assure care plans and goals are being followed. Requirements A valid Occupational Therapy license in NYS. Strong knowledge of medical terminology and case management principles. Experience working with diverse populations. Familiarity with is preferred. Excellent communication skills, both verbal and written, to effectively interact with patients, families, and team members. Ability to work independently as well as part of a multidisciplinary team. Previous experience in discharge planning is a plus.
